Make a referral enquiry

When you make a referral enquiry, we begin with the person’s choice, goals and preferences. Basecare provides registered NDIS support across Melbourne, including Hampton Park and the south-east.

What helps us start the conversation

With the participant’s consent, it can be helpful to share:

  • the person’s preferred contact method and who they would like involved in the conversation
  • the kind of support they are looking for and the goals that matter to them
  • their suburb, preferred days or times and any accessibility considerations
  • how their plan is managed, where relevant
  • any information that will help us understand whether Basecare may be a suitable fit

Please only share information that is necessary for the referral conversation and make sure the participant has agreed to the referral.

What happens next

  1. We listen first. We discuss the person’s goals, preferences and the type of support they are seeking.
  2. We explore fit. We talk openly about the services Basecare provides, availability and whether the support is likely to be suitable.
  3. We agree on next steps. If Basecare is a good fit, the participant can decide whether they would like to move forward with a service agreement.
