Preparing for an NDIS plan reassessment can feel like a lot. Starting early and keeping your information together can make it easier to explain what is working, what has changed and what support you need next.
Start with your current plan
Look at your current goals and the supports you have been using. Think about what has helped, what has not worked as expected, and whether your disability support needs, living situation or goals have changed.
Gather useful information
The NDIS recommends gathering information and evidence that explains your current situation and support needs. This may include reports from treating health professionals, assessments, progress information and notes about the everyday impact of your disability.
Write down what you want to discuss
- Changes in your daily life, health, home, work or study.
- Goals you are working towards now and goals you want to pursue next.
- Supports that are helping you build independence or participate in the community.
- Questions you would like to ask during your check-in or reassessment.
What happens next?
Your my NDIS contact may arrange a check-in before your reassessment date. The NDIA will consider the information and evidence you provide when deciding whether to vary your plan, make a new plan or keep your current arrangements.
